Isoproterenol (Isoprenaline) is a adrenergic agonist (direct acting), used as cardiac stimulant in treating certain heart problems (eg, heart attack, congestive heart failure), blood vessel problems (eg, shock), and certain types of irregular heartbeat (eg, heart block).
Dose : 1-2 mg I.M. , 5-10 μg/min. I.V. Infusion, 20 mg sublingual
Route : Intravenous, Intramuscular, Enteral (sublingual), Inhalation
Plasma Half-life : ~2 minutes
Duration of action : 1-3 hours

Therapeutic uses : Isoprenaline has following usage :

  • May be used in emergencies to stimulate heart rate in patients with bradycardia or heart block, particularly in anticipation of inserting an artificial cardiac pacemaker or in patients with the ventricular arrhythmia torsades de pointes i.e. Occasionally used to maintain Idioventricular rate till pacemaker is implanted.
  • Maybe used as bronchodilators to treat bronchial asthma.

Point of interest : Its nonselectivity is one of its drawbacks and the reason why it is rarely used therapeutically.
Side effects :  Adverse effects of Isoprenaline are as follows:

  • Palpitations,
  • Tachycardia,
  • Headache,
  • Flushing,
  • Cardiac ischemia,
  • Sinus tachycardia,
  • Arrhythmias.

Contraindications :

  • Angina, pre-existing ventricular arrhythmias, tachyarrhythmias; cardiac glycoside intoxication.
