Ceftaroline is the advanced generation or fifth generation cephalosporins. It is a broad spectrum antibiotic that is administered as a prodrug, ceftaroline fosamil.

Antibacterial spectrum : Ceftaroline has broad gram-positive activity. it also has similar gram-negative activity to the third-generation cephalosporin ceftriaxone.

  • Important gaps in coverage include P. aeruginosa, extendedspectrum β-lactamase (ESBL)-producing Enterobacteriaceae, and Acinetobacter baumannii.
  • It has activity against MRSA. The unique structure allows ceftaroline to bind to PBP2a found with MRSA and PBP2x found with Streptococcus pneumoniae. It is the only commercially available β-lactam in the United States with activity against MRSA.

Therapeutic uses : Ceftaroline is indicated for the treatment of complicated skin and skin structure infections and community-acquired pneumonia.
Mechanism of action : Cephalosporins cause inhibition of bacterial cell wall synthesis. i.e. they have the same mechanism of action as penicillin. However, they bind to different proteins than those which bind penicillins.
Side effects : Cephalosporins are generally well tolerated, but are more toxic than penicillin. Ceftaroline may provoke following side effects :

  • Positive direct coombs test,
  • Diarrhea,
  • Nausea,
  • Nosebleeds,
  • Loss of appetite,
  • Dark urine,
  • Insomnia,
  • Pruritus, Rash.

Warnings & Precautions

  • Hypersensitivity reactions
  • Superinfection (Clostridium difficile-associated diarrhea)
  • Development of drug-resistant bacteria
  • Direct Coombs test seroconversion

Drug Interactions : Ceftaroline may interact with following drugs :

  • BCG (Intravesical) : Antibiotics may diminish the therapeutic effect of BCG (Intravesical).
  • BCG Vaccine (Immunization) : Antibiotics may diminish the therapeutic effect of BCG Vaccine (Immunization).
  • Cholera Vaccine : Antibiotics may diminish the therapeutic effect of Cholera Vaccine.
  • Lactobacillus and Estriol : Antibiotics may diminish the therapeutic effect of Lactobacillus and Estriol.
  • Probenecid : May increase the serum concentration of Cephalosporins.
  • Sodium Picosulfate : Antibiotics may diminish the therapeutic effect of Sodium Picosulfate.
  • Typhoid Vaccine : Antibiotics may diminish the therapeutic effect of Typhoid Vaccine.

Contraindications :

  • Contraindicated in patients having known serious hypersensitivity to ceftaroline, other members of the cephalosporin class, or any component of the formulation.
