Definition

  • Failure To Thrive used for infants & childrens upto 5 year of age whose physical growth is significantly less than their peers of same age & sex.

FTT usually refers to

  • Weight for age below 3rd centile OR
  • Failure to gain weight over a period of time OR
  • A change in rate of growth that has crossed two major centiles e.g. 75th to 50th, over a period of time.

Weight of the child

A. Weight formula (Weech’s formula)

If child is 3-12 months

  • Expected weight (kg) : Age (months) + 9 / 2

If child is 1-6 year

  • Expected weight (kg) : [Age (years) × 2] + 8

If child is 7-12 year

  • Expected weight (kg) : [Age (years) × 7] – 5 / 2

Ready reckoners

If birth weight is X then

  • At 5 months ⇒ 2x
  • At 1 year ⇒ 3x
  • At 2 year ⇒ 4x
  • At 3 year ⇒ 5x
  • At 5 year ⇒ 6x
  • At 7 year ⇒ 7x
  • At 10 year ⇒ 10x

% gain : (3 x- x / x ) × 100 = 200%

B. Height or Length formula

  • At birth: 50 cm
  • At 1 year: 75 cm
  • At 2 year: 87.5 cm
  • At 4 year: 100 cm

Expected height (cm) between 2-12 year : [ Age (years) × 6 ] + 77

C. Head circumference & Chest circumference

At birth

  • Head circumference: 34 cm
  • Chest circumference: 31 cm

At 9 month of age

  • Head circumference = Chest circumference

At or beyond 12 month of age

  • Chest circumference > Head circumference

Note: If gap between head circumference & chest circumference at the birth is greater than 3 cm, it suspect intrauterine growth restriction (IUGR).

D. Upper segment to lower segment ratio (US:LS)

  • At birth: 1.7:1 to 1.9:1
  • At 3 years: 1.3:1
  • At 7 years: 1.1:1
  • At 10 years: 1:1
  • Beyond 10 years: 0.9:1 ⇒ 1:1.1
